State Charitable Solicitation Disclosures

Certain states require written disclosures for nonprofit organizations soliciting contributions. Individual state disclosures are below.

Florida:

A COPY OF THE OFFICIAL REGISTRATION AND FINANCIAL INFORMATION MAY BE OBTAINED FROM THE FLORIDA DIVISION OF CONSUMER SERVICES BY CALLING TOLL-FREE WITHIN THE STATE 1-800-HELP-FLA (1-800-435-7352) OR BY VISITING www.FloridaConsumerHelp.com. REGISTRATION DOES NOT IMPLY ENDORSEMENT, APPROVAL, OR RECOMMENDATION BY THE STATE. Florida Registration #: CH33791. 100% of each contribution is received by the Foundation for the National Institutes of Health, Inc., a Maryland 501(c)(3) not-for-profit organization.

Georgia:

A full and fair description of the FNIH’s programs and activities and a financial statement will be sent upon request, which request may be submitted to FNIH’s office located at 11400 Rockville Pike, Suite 600, North Bethesda, Maryland 20852, telephone (301) 402-5311.

Maryland:

A copy of the FNIH’s current financial statement is available upon request, which request may be submitted to FNIH’s office located at 11400 Rockville Pike, Suite 600, North Bethesda, Maryland 20852, telephone (301) 402-5311. For the cost of copies and postage, documents and information submitted by the FNIH are available from the Maryland Secretary of State, 16 Francis Street, Annapolis, Maryland 21401.

Mississippi:

The official registration and financial information of the Foundation for the National Institutes of Health, Inc. may be obtained from the Mississippi Secretary of State’s office by calling 1-888-236-6167. Registration by the Secretary of State does not imply endorsement by the Secretary of State.

Nevada:

Contributions to the FNIH may be tax deductible pursuant to the provisions of section 170(c) of the Internal Revenue Code of 1986, 26 U.S.C. § 170(c).

New Jersey:

INFORMATION FILED WITH THE ATTORNEY GENERAL CONCERNING THIS CHARITABLE SOLICITATION AND THE PERCENTAGE OF CONTRIBUTIONS RECEIVED BY THE FNIH DURING THE LAST REPORTING PERIOD THAT WERE DEDICATED TO THE CHARITABLE PURPOSE MAY BE OBTAINED FROM THE ATTORNEY GENERAL OF THE STATE OF NEW JERSEY BY CALLING 973-504-6215 AND IS AVAILABLE ON THE INTERNET AT http://www.state.nj.us/lps/ca/charfrm.htm. REGISTRATION WITH THE ATTORNEY GENERAL DOES NOT IMPLY ENDORSEMENT.

New York:

Upon request, a person may obtain from the FNIH or from the New York Attorney General a copy of the last financial report filed by the FNIH with the Attorney General. Such request should be addressed to the FNIH at 11400 Rockville Pike, Suite 600, North Bethesda, Maryland 20852, or to the Attorney General at 28 Liberty Street, New York, NY 10005, telephone (212) 416-8401, or www.charitiesnyc.com. North Carolina:

Financial information about the FNIH and a copy of its license are available from the North Carolina State Solicitation Licensing Branch at (919) 814-5400. The license is not an endorsement by the State.

Pennsylvania:

The official registration and financial information of the Foundation for the National Institutes of Health, Inc. may be obtained from the Pennsylvania Department of State by calling toll-free, within Pennsylvania, 1-800-732-0999. Registration does not imply endorsement.

Virginia:

Financial statements are available upon request from the State Division of Consumer Affairs, Department of Agricultural and Consumer Services, P.O. Box 1163, Richmond, VA 23218.

Washington:

The registration required by the Washington Charitable Solicitation Act is on file with the Secretary of State’s office. A donor can obtain additional financial and other information from the Washington Secretary of State at (800) 332-4483 or https://www.sos.wa.gov/charities/. The FNIH is not part of any unit of government.

West Virginia:

West Virginia residents may obtain a summary of the registration and financial documents from the Secretary of State, State Capitol, Charleston, West Virginia 25305. Registration does not imply endorsement.
